Volunteer Application

I understand that Eleventh Hour Rescue may not be able to accept all individuals who would like to volunteer, and that Eleventh Hour Rescue retains discretion to accept or decline volunteer requests based on the needs of the program. I understand that submitting this application form does not automatically register me as an Eleventh Hour Rescue volunteer, and that there may be certain qualifications I must meet, including the acceptance of established volunteer policies and procedures before I may begin volunteering. I understand that I must sign a liability waiver prior to the start of volunteering agreeing that Eleventh Hour Rescue shall not be held responsible for any damages, costs, or expense resulting from volunteering or animal interactions; including, but not limited to, damage or injury to persons property. In this connection, Eleventh Hour Rescue is held harmless from any and all liability of any and every nature and cause, directly and indirectly relating to volunteering or animal interactions. By submitting this form, I confirm that, (a) I am 18 years of age or older, (b) I have reviewed and meet the VOLUNTEER ESSENTIALS required of all volunteers, (c) I give permission to Eleventh Hour Rescue to contact my references, (d) I give permission to Eleventh Hour Rescue to contact my veterinarian(s) to obtain information about my current and previous pets, (e) I give permission to Eleventh Hour Rescue to verify the information above, (f) and I attest that the information I have provided on this form is true and accurate.
